Findings
According to documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan while assisting him/her with a shower. The failure resulted in AP2 using physical force against AV causing fear and discomfort, which is considered physical abuse. The facility failed to protect AV from abuse, which is a violation of Oregon Administrative Rules.
