Findings
On or about March 17, 2021, Alleged Perpetrator 2 (AP2) failed to follow Alleged Victim's (AV) care plan when he/she transferred AV with a Hoyer lift from AV's wheelchair to AV's bed without assistance from another staff member. AV is care planned as a 2 person Hoyer lift assist. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
