Findings
On or about May 8, 2021, Alleged Victim (AV) was given h/her medications and another residents medication by Alleged Perpetrator (AP2) putting AV at risk for harm. AP2 actions are a violation of residents’ rights is neglect of care and constitutes abuse. The facility failed to protect AV from risk of harm, which is a violation of Oregon Administrative Rules.
