- Alleged Victim (AV) had a known history of touching others in a reassuring manner and Witness 1 (W1) had a known history of aggressive behaviors. On or about May 16, 2021, AV touched another resident which resulted in that resident grabbing AV and throwing him/her to the ground. AV was transferred to the hospital and diagnosed with a hip fracture. AV died May 28, 2021 due to the traumatic nature of the hip fracture. The facility failed to appropriately care plan regarding AV's known behaviors and provide a safe environment, which is neglect of care and constitutes abuse.
