Findings
Alleged Victim has a known history of elopement. On or about May 18, 2021, AV went out the facility door and broke the courtyard fence and eloped from the facility. The facility door alarm leading out to the courtyard and fenced area did not go off. The facility failed to provide AV with a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
