Findings
Alleged Victim (AV) relies on the facility for his/her care. The facility failed to provide a safe medication administration system to ensure AV medication was available to administer. AV was not administered the prescribed amount of insulin on or about May 19, 2021 due to lack of supply of insulin in stock. The facility failed to reorder AV insulin supply prior to running out. This put AV at risk of serious harm which is a violation of resident’s rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(ii).
