Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about May 19, 2021, AV fell in the common are resulting in a laceration to the back of his/her head which required five (5) staples to repair. AV was hospitalized for five (5) days due to a brain hemorrhage. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
