Findings
The Alleged Victim (AV), Witness #1 (W1) and Witness #4 (W4) had multiple altercations over approximately 2 months. During that time, AV suffered hits, slaps, hair pulling, ear pulling, thrown against a wall and his/her wrists grabbed. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
