- The facility failed to provide appropriate services and follow the Alleged Victim’s (AV) care plan when his/her condition changed. Facility documents indicate that staff should take AV’s blood sugar readings if he/she is showing signs of low or high blood sugar, such as fatigue, shakiness, dehydration, dry mouth, or increased confusion and they are required to arrange for emergency medical transport for a resident showing signs of unconsciousness or inability to waken easily. Between approximately May 18, 2021, and May 22, 2021, AV declined most or all of their food, refused care, and did not want to get up. AV’s blood sugar was not checked despite AV’s decline and refusal of all food for approximately two (2) days. On or about May 22, 2021, AV was sent to the hospital where AV was diagnosed with dehydration, diabetic ketoacidosis, and acute hyperglycemia. AV passed away a few days later. The facility failed to provide appropriate services and respond to AV’s change of conditions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
