Findings
Alleged Perpetrator 2 (AP2) failed to provide basic care and services to Alleged Victim (AV) which resulted in AV falling and hitting his/her head when AP2 turned his/her back on AV while assisting AV with his/her care needs. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
