Findings
On or about May 25, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation that resulted in an injury to W1's nostril and injuries to AV's hands. The facility failed to provide a safe environment for W1 by failing to protect W1 from AV's aggressive behaviors which is a violation of resident rights, is considered neglect of care and constitutes abuse.
