Findings
On or about May 26, 2021, facility staff discovered that Alleged Victim (AV) had an unexplained bump on the back of his/her head. An investigation determined that AV suffered multiple falls while under facility supervision and that the facility failed to provide appropriate oversight and monitoring of AV's risk for falls. The facility's failure to properly care plan for AV's fall risk is a violation of resident rights, is considered neglect of care and constitutes abuse.
