Findings
Witness #1 (W1) is know to be easily agitated and has frequent altercations. On or about May 27, 2021, W1 left his/her chair in the dining room, the Alleged Victim (AV) straightened the chair and W1 returned and began pushing AV. The two pushed each other until AV fell, hitting his/her head. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
