Alleged Victim (AV) relies on the facility for h/h care. AV requires assistance with ambulation. On or about May 29, 2021, AV was not provided assistance with ambulation due to lack of staff which caused a resident-to-resident incident. Failing to assist with ambulation is a violation of resident’s rights, is neglect of care and constitutes abuse.
