Findings: Alleged Victim (AV) relies on the facility for all his/her care needs. It has been observed on a visit to AV on or about June 1, 2021 that AV’s hair was disheveled and has not been assisted with toileting. AV’s room is only cleaned once a week. AV’s bathroom had a soiled spot near toilet, soiled clothing, towels, and blankets. AV had multiple drinks on the table near his/her chair. AV’s room smelled of urine and feces and urine and feces were visible in a commode near AV’s chair. Progress notes did not show staff reporting refusal of showers or concerns regarding grooming. The facility failed to provide service to AV which is a violation or resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP21-03090 $250.00 fine assessed
