Findings
On or about April 19, 2021, the Alleged Victim (AV) was being assisted in the restroom by Alleged Perpetrator #2 (AP2) when AP2 was overheard stating a derogatory remark to AV. AP2's actions are a violation of resident rights, are considered neglect of care and constitute verbal abuse. The facility failed to protect AV from verbal abuse, which is a violation of Oregon Administrative Rules.
