- Alleged Victim (AV) has a history of getting fevers and going to the hospital with a urinary tract infection (UTI). On or about May 13, 2021, AV’s pendent was pushed at 6:51 AM and 10:21 AM so that his/her temperature would be taken. AV’s temperature was not taken until around 11:15 AM which was 103.7. AV was transported to the hospital and treated for septic shock from a UTI. The facility failed to answer the call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
