Findings
On or about June 1, 2021, the Alleged Victim (AV) called on his/her call button for assistance. Alleged Perpetrator #2 (AP2) answered the call and proceeded to take AV to the restroom. AV is care planned for a one person transfer with a gait belt. Ap2 transferred AV without the gait belt and AV fell in the bathroom, sustaining bruising in multiple locations. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to ensure the care plan was being followed, which is a violation of Oregon Administrative Rules.
