Findings
The facility failed to ensure supervision and staff support regarding known behaviors and altercations related to the Alleged Victim (AV) and Witness #1 (W1). An incident occurred between the two where W1 and AV were slapping each other during an altercation. AV is care planned to be kept away from W1. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
