Findings
On or about May 3, 2021, it was noted that the Alleged Victim (AV) had a rash on both feet. AV's primary care physician (PCP) was contacted, however, there was no response. On or about May 12, 2021, AV's rash was still on his/her feet, and AV was wearing non-skid socks with shoes, which AV is care planned not to wear non skid socks with shoes. AV's shoes were tight, causing marks to AV's feet. On or about May 13, 2021, AV's PCP was contacted again with no response. On or about June 1, 2021, AV's PCP was contacted once again, and the treatment for AV's rash began on June 7, 2021. The facility failed to assure timely medical treatment for AV, causing AV unnecessary pain and discomfort. The facility's failure is a violation of residents rights, is considered neglect of care and constitutes abuse.
