- On or about June 4, 2021, the Alleged Victim (AV) was admitted to the hospital with increased sodium levels due to severe dehydration. The facility was to give AV hydration via a spill proof cup, however, AV had a difficult time getting liquid from the cup, so staff added a straw, however, there is no documentation if this action helped AV to stay hydrated. Because AV was dehydrated when admitted to the hospital, it indicates that AV was not getting proper hydration. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
