Findings
On or about June 8, 2021, the facility failed to provide a safe environment by failing to assist with supervision for safety which resulted in Witness 1 (W1) pushing Alleged Victim (AV) to the floor. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
