- Alleged Victim (AV) is care planned to receive catheter care and oversight with cleaning and replacement. AV was taken to the hospital or urgent care for abdominal/pelvic pain, acute infection and/or blood in urine on or about May 24th, June 8th, 11th, and 14th, 2021. Facility staff had reported concerns with inadequate catheter care went on for months, but no new or different interventions or strategies were put in place. Staff noted frequent catheter care concerns, such as blood in urine, but no actions were taken to address concern. The facility failed to follow AV’s care plan around AV’ catheter care and cleaning, which is a violation of resident rights, is neglect of care and constitutes abuse.
