Findings
The Alleged Victim (AV) had multiple falls in early June as AV was declining. The facility failed to provide staffing based interventions to alleviate AV's falls. AV had a fall on June 10, 2021, in which AV was sent to the hospital. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
