- Alleged Victim (AV) is care planned to have staff supervision when he/she is outside. Alleged Perpetrator 2 (AP2) is aware AV needs supervision when he/she is outside. On or about June 8, 2021, AV was left outside by AP2 and left AV alone. AV fell and suffered some bruising. AP2 failed to follow the care plan, which is neglect of care and constitutes abuse. The facility failed to assure AV’s care plan was being followed, which is a violation of Oregon Administrative Rules.
