Findings
Alleged Victim (AV) relies on the facility for h/her care. On or about June 10, 2021, AV wandered into Witness 1 (W1) room, and W1 hit AV on the head with h/her cane. The facility failed to provide a safe environment putting AV at risk of harm, which is a violation of residents rights is neglect of care and constitutes abuse as defined in OAR 411-020-0002(1)(b)(A)(ii).
