- Alleged Victim (AV) cannot ambulate independently and requires assistance from two persons to transfer while using a Hoyer lift. There is not always two staff members available to help AV when he/she requires assistance with transfers. It is difficult and painful for AV to sit up in the shower chair. AV always requires physical assistance to complete bathing tasks and has a caregiver from an outside agency who assists AV with showers, but this outside provider is not trained on how to operate the Hoyer lift. The facility failed to provide service as needed for AV which is a violation of resident rights, is neglect of care and constitutes abuse.
