Findings
Alleged Victim (AV) relies on the facility for his/her care. AV was care planned for ongoing monitoring and follow-up on skin issues. According to AV, witnesses and documents, the facility failed to adequately assess and monitor AV's leg infection and failed to ensure transportation to a medical appointment. AV experienced pain and discomfort and required hospital treatment. These failures are a violation of resident rights, is considered neglect of care and constitutes abuse.
