- The facility failed to provide a service for Alleged Victim's (AV) by failing to ensure AV's care plan was updated timely, as AV's care needs increased which resulted in a delay in AV being provided with doctor ordered nectar thick liquids for safety. An investigation also determined that there was at least one incident in which AV did not receive incontinence care for an extended period of time resulting in AV lying in a wet brief that soaked through to his/her bedding. The facility's failures is a violation of resident rights, is considered neglect of care and constitutes abuse.
