Findings
Alleged Victim is care planned to be provided with a 1 on 1 supervision when out in the courtyard due to elopement behaviors. On or about June 18, 2021, AV entered the courtyard and made the alarm door go off. AV then eloped from the facility by climbing over the fence in the courtyard. Alleged Perpetrator 2 (AP2) didn’t call for assistance from other staff when he/she knew AV made the door alarm go off. AP2 was aware that AV was in the courtyard. AP2 failed to follow AV’s care plan, which is neglect of care and constitutes abuse. The facility failed to assure AV’s care plan was been follow, which is a violation of Oregon Administrative Rules.
