Findings
Alleged Victim (AV) relies on the facility for his/her care. On or about June 21, 2021, AV eloped from the memory care unit and no one was aware until AV was brought back by staff that happened to see AV walking by. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
