Findings
On or about June 20, 2021 at approximately 10:00 pm, the Alleged Victim (AV) eloped from the facility and walked down a busy road to where AV entered a business. The facility was contacted by the Police that AV was found and AV was brought back to the facility. AV was unharmed, however, AV leaving the facility unassisted placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
