Findings
On or about April 23, 2021, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation due to AV entering W1's room resulting in W1 being shoved to the ground causing W1 a head injury and fractured wrist. The facility failure to provide a safe environment for W1 is a violation of resident rights, is considered neglect of care and constitutes abuse.
