Findings
On or about March 17, 2021, the Alleged Victim (AV) ingested shampoo. AV stated it was because he/she wanted to kill him/herself. AV was placed on safety checks at 16 times per shift. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
