- Alleged Victim (AV) relies on the facility for h/h care. On or about June 24, 2021, AV's care plan was not followed, and medication was left at his/her bedside, which resulted in AV dropping medication. According to a witness, AV was known to have to difficulty maneuvering the recliner. After dropping medication, the AV used a remote to lift the hydraulic footrest on the recliner to look for the medication which caused AV to fall. AV experienced pain, swelling and discoloration for days after the fall. The facility failed to ensure care plans were followed and failed to care plan for AV's known difficulty with the recliner, both of which are violations of resident rights, is neglect of care which constitutes abuse.
