Findings
On or about June 27, 2021, Alleged Perpetrator 2 (AP2) failed to provide a safe environment for Alleged Victim (AV) by failing to administer AV's pain medication as ordered resulting in AV experiencing pain and discomfort. AP2's actions is considered neglect of care and constitutes abuse. The facility's failure to provide a safe environment is a violation of Oregon Administrative Rules.
