Findings
On or about June 26, 2021, Alleged Perpetrator 2 (AP2) failed to follow The Alleged Victim (AV) care plan to help with toileting, and transfers as needed. AV was found by morning staff, approximately two and half hours after AP2 helped AV. The failure resulted in AV suffering pain, and unreasonable discomfort. AP2's actions is a violation of resident rights, is considered neglect of care and constitute abuse. The facility failed to ensure care plans were followed, which is a violation of Oregon Administrative Rules.
