Findings
On or about May 18, 2021, Alleged Perpetrator #2 (AP2) was observed using physical force to remove the Alleged Victim (AV) from his/her room. AV did not want to leave his/her room and was visibly upset by AP2 forcing him/her to leave, causing unnecessary emotional distress. AP2's actions are a violation of resident rights, is considered neglect of care and constitutes abuse. The facilities failure to provide a safe environment is a violation of Oregon Administrative Rules.
