Findings
According to documentation the facility failed to follow the Alleged Victim (AV) care plan to assist with meal intakes. Between On or about April 15, 2021, and June 28, 2021, AV was not assisted with meals. The failure resulted in AV losing weight from not being able to eat, and AV experiencing pain and unreasonable discomfort. The facility failed to ensure care plans were followed, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
