Findings
Alleged Victim (AV) has a history of falls while attempting self-transfers and is care planned for a tab-alarm. On or about, July 1, 2021, AV was found on the floor with a head injury and was transported to the hospital. According to witnesses, AV's tab-alarm batteries had died. The facility failed to ensure the tab alarm was functioning properly, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
