- Alleged Victim (AV) has a known history of falls. Between April 22nd through July 5th, 2021, AV sustained approximately seven (7) falls, AV sustained injuries in at least six of those falls. On or about July 7, 2021, staff responded to a call light and found AV on the bathroom floor of his/her apartment. AV was located halfway in his/her bathroom and hallway in the entry way, AV sustained a skin tear to his/her right elbow. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
