Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to be checked every 2 hours for incontinence care. On or about July 03, 2021, AV was told to stay in his/her bed as the facility was short staffed and later in the day was found in his/her bed with undergarments and bedding soaked in urine, causing unreasonable discomfort. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
