Findings
Alleged Victim (AV) relies on the facility for his/her care. AV is care planned for transfer assist, and remain with AV while toileting. On or about July 03, 2021, AV was left unattended while toileting during change of shift and fell from commode resulting in injury. The facility failed to follow the care plan which is a violation of resident’s rights is neglect of care and constitutes abuse.
