Findings
The facility failed to provide appropriate services according to AV's needs for activities of daily living. The failure resulted in AV being discovered in soiled briefs, not showered according to his/her care plan and had complaints about lack of assistance with transfers, causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
