Findings
Alleged Victim (AV) has a known history of fear and a tendency to grab at the sling during transfers. On or about, July 1, 2021, the AV received a bruise and swollen during a hoyer transfer. The facility failed to appropriately care plan regarding AV's fears and tendency to grab at the sling during transfers which is a violation of resident rights, is neglect of care and constitutes abuse.
