Findings
On or about July 12,2021, it was reported that the Alleged Victim (AV) suffered a skin tear to his/her inner right thigh. It was thought that the skin tear was caused by long finger nails, belonging to Alleged Perpetrator #2 (AP2), however, AP2 denies having long nails at the time of giving cares to AV. An investigation determined no wrongdoing or abuse by AP2. It is undetermined how AV received the skin tear. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
