Findings
Alleged Victim (AV) relies on the facility for h/h care. AV has a history of bruising. On or about July 12, 2021, AV was found to have bruises of to the top of AV's right and left hand. The facility failed to ensure a safe environment, which is a violation of resident’s rights, is neglect of care and constitutes abuse.
