Findings
Alleged Victim (AV) relies on the facility for his/her care. Witness 1 (W1) has been verbally aggressive and threatening towards AV on May 9th, 10th and 29th, 2021 and on July 5, 2021. On or about July 13, 2021, W1 was screaming at AV and AV screamed back, W2 came out of his/her room yelling “unrecognizable words” charging towards AV, then hit AV on the shoulder. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
