Findings
The facility failed to implement interventions and appropriately care plan related to AV's risk of falls. The Alleged Perpetrator 2 failed to appropriately transfer AV, resulting in an injury fall. The facility and AP2 failure caused injury and unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
