Findings
The Alleged Victim (AV) has a history of wandering and exit seeking. On or about July 14, 2021, AV was found outside of the facility without supervision, and it is unknown how AV left the facility. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
